MODUS MUSIC ORDER FORM

We will ship up to 5 items from our Catalogue per Order Form, multiple forms may be submitted.

Please complete the form as fully as possible, including your e-mail address if you wish to receive an acknowledgement of your order.
Please use 'Tab' key to move cursor between fields, using 'Enter' will submit the Form.

Any special information (eg. requirement for 'Performing Set' or 'Large Score and Parts', etc.)
should be included in the optional 'additional information' text box.

When you are satisfied with your completion of the form click Send Order.
Click Clear Form to delete all form contents (and start over?).

At present we cannot calculate here the total cost of your order, please note that a Postage & Packing charge will be applied.
Note also that prices shown here are correct and may differ slightly from our printed catalogue prices.
An Invoice will be forwarded with your order, early settlement would be appreciated.

We aim to despatch your order within 10 days of receipt. If this is not possible you will be contacted via e-mail, telephone or letter.
